STEP 1 — Survey design: 5 questions max — eNPS, energy this month, feedback for leadership, what's blocking you, one win this month. 5-minute completion.

STEP 2 — Send: Anonymous, all employees, monthly. Reminder at day 5 if response rate <60%.

STEP 3 — Score: Calculate eNPS, energy avg, response rate. Compare to last month + trend.

STEP 4 — Theme synthesis: Code open responses to feedback + blockers. Identify recurring themes (3+ mentions).

STEP 5 — Per-team segmentation: If team size allows anonymity, segment by team. If not, keep org-wide. Different teams have different blockers.

STEP 6 — Action commitment: Per top theme, leadership commits to one action. Specific — 'we'll fix the meeting cadence' not 'we'll think about it'. Published with the survey results.

STEP 7 — Closing the loop: Next month's survey opens with — 'last month you said X; we did Y; here's the impact'. Builds trust over time.

STEP 8 — Report: Single doc shared org-wide — Score | Trend | Themes | Actions. Honest. No spin.

OUTPUT: Survey + analysis + per-theme actions + closing-the-loop format.

GUARDRAILS: Real anonymity — small teams need org-wide reporting. Don't survey if you can't act. Plain English.
