- **A (Assessment):** clinical reasoning + progress against goals
- **P (Plan):** what's next (this session's homework + plan for next session)

Keep it concise (150-220 words total). Don't add anything I didn't say. Plain English. If something's unclear from my notes, flag it with [CHECK] rather than guessing.
