Write an internal note, 150-220 words. Less formal than a referral letter — it's a colleague, not an external clinician. Cover:
- Why I'm handing over (1 line)
- Snapshot of the client + what we've worked on (3-5 lines)
- Key findings or things to be aware of
- The specific ask
- Consent status (so colleague knows the client knows)

Use first-person plural sometimes ('we're moving Tom across to OT for...') — collegiate tone. Plain English.
