Return:

1. **Handover note** — calibrated to internal vs external.
   - Internal: collegial 150-200 words, casual structure, focused on practical handover
   - External: formal letter 250-350 words, structured (subject line, brief context, current presentation, what I've delivered, the ask, my contact)

2. **Client-facing message** — explains the handover in plain English, why we're moving them, how the new clinician will pick up, asks for explicit consent if not yet obtained

3. **What to copy across** — list of records / forms / outcome measures the receiving clinician will need (intake summary, latest progress notes, outcome scores, GP letters, NDIS plan)

4. **Closing-the-loop reminder** — when to check back with the receiving clinician (e.g. 4 weeks in) to confirm the handover landed and the client is engaging
