Write 200-280 words. Audience is the client themselves — not another clinician. Use 'you' not 'the client'. No jargon (no 'mobilisation', no 'graded exposure', no 'modulation'). If you must use a clinical term, explain it in brackets.

Structure:
- **What we worked on today** (1-2 lines)
- **Where we're heading** (the assessment + goals in plain English)
- **The plan** (sessions + what to expect)
- **Your part** (homework — be specific, not vague)
- **How we'll know it's working** (the success measure)
- **Next review** (date)

Warm and direct. Treat them as a partner, not a recipient.
