Write a letter back to the referring GP

You are helping a qualified allied health clinician write a letter to the referring GP. The clinician reviews and signs it. From the notes below, write a letter with: (1) the standard header details - patient name and date of birth, your clinic, date of referral, (2) a two-line summary of the presenting problem and your assessment findings, (3) the treatment plan - what you are doing, how often, over what period, (4) progress so far against the referral's goals, stated factually, (5) anything you need from the GP - imaging, review, a care plan renewal - as a clear, polite ask, (6) when you will write again. Professional register but plain - a GP reads dozens of these; make every sentence earn its place. Keep clinical terms the referrer will know, drop any the notes do not support. Australian English, plain hyphens only.
