End-of-day notes tidy-up that stays on your phone
Your dictated notes get cleaned into your usual structure on the drive home, with nothing leaving the device.
Set this up once, then it runs on its own
- Runs
- daily, 6pm
- Needs
- an iPhone with Apple Intelligence, on iOS 26 or later. No subscription.
- Setup
- about fifteen minutes, once
WHY THIS ONE IS BUILT DIFFERENTLY
Every other loop in this library sends your words to a server. This one does
not. It uses the model built into your phone, so your patients' information
never leaves the device, never reaches a company, and never reaches us. That is
the whole point of it.
BEFORE YOU SET IT UP
Two things that are not optional:
- Consent. You need your patient's consent before using any AI tool on
their information, re-confirmed at each subsequent consult and noted in the
record. That is an Ahpra requirement, and it applies even though this runs on
your own phone.
- This tidies, it does not think. The prompt below restructures your own
words and nothing more. Do not change it to suggest a diagnosis, an
impression or a treatment. A tool that only transcribes and formats is not a
medical device. One that analyses or interprets is, and that is a different
regulatory world entirely.
SETUP
- Open Shortcuts, make a new shortcut, and add the Use Model action.
- Tap the model and choose On-Device. Not Private Cloud Compute, not
ChatGPT. On-Device is the one that works with no network.
- Set the input to the note you have been dictating into during the day.
- Paste the prompt below as the instruction.
- Go to the Automation tab, add a Time of Day automation for 6pm, and
point it at this shortcut. Then turn Ask Before Running off and **Run
Immediately** on, or it will just sit there waiting for you to tap it.

Updates live as you typeYou are tidying up my own {{discipline}} session notes. Take the rough notes
below and lay them out under these headings: {{structure}}.
Rules:
- Use only what is in my notes. Do not add anything.
- Do not add an impression, a diagnosis, a differential or a suggested
treatment, even if my notes seem to point at one. If I did not write it, it
does not go in.
- Expand my shorthand into full clinical terms where it is obvious, and leave it
alone where it is not.
- Keep my clinical reasoning in my words.
- If something is unclear or looks like it is missing, list it at the end under
"Check before filing" rather than filling the gap.
- Output the structured note and nothing else. No preamble.Fill in the required fields above to copy or run this prompt.
Turning it off
Read what it gives you before it goes in the file. You are responsible for the
note, not the phone.
This prompt produces general information, not medical or clinical advice. Apply your own professional judgement, or see a qualified health practitioner.
